Next Steps: 

  1. Print two copies of the form. If you check the appropriate box about needing a printed version, we’ll mail you a copy of the form.
  2. Sign and date the forms.
  3. Mail one of the signed forms to the address at the top of the form. We highly recommend sending it via certified mail.
  4. Mail a second signed form to your local association. Links to the contact information for most CTA locals is available on the CTA website.
  5. Send a copy of the form to your employer’s payroll officer and keep a copy for your files.

Keep a copy of the letter for your reference.

If you have requested to receive a hard copy of your form in the mail, we will send it to you.
